Captain Nomination Form

To be a captain requires a higher level of service, commitment, and sacrifice.

High School Student-Athletes

To create an intentional experience and because space is limited, participation in Captains Camp is based on a nomination process. 
How is works:
Step #1: Because of what a coach sees or believes in his/her athlete(s), they nominate him/her on this page.

Step #2: The nomination goes to our local FCA staff to review and consider.

Step #3: Email notification is sent to the coach of acceptance or denial. 

Step #4: If accepted, registration instructions and a scholarship code is sent to the coach to inform and pass on to the athlete.
